What happens after I use my included visits?

Are medications included?
No. Prescription and refill management may be provided when medically appropriate, but medication costs are not included.

No. Revitalize Care is designed for routine, preventive and ongoing primary care and does not include those services.
How do I join Revitalize Care?
Submit the enrollment form below. Our team will contact you to confirm your membership option, complete the membership agreement and activate recurring payment. Membership becomes active after the agreement is signed and the initial payment is successfully processed.
When is the preventive lab panel available?
Annual members may use the included preventive lab benefit after membership activation. Monthly members become eligible after 90 consecutive days of active membership in good payment standing.
How do I cancel my membership?
Members may submit a cancellation request through their Square subscription management page or provide written notice to Revitalize. Membership termination and any applicable refund are handled according to the Membership Agreement.
